Festival Title Authorization Form

Contestants who wish to represent a festival or community event title (ex: Strawberry Festival, Pumpkin Festival, Three Rivers) must receive permission from the festival to use the name during the Miss West Virginia USA® or Miss West Virginia Teen USA® State Pageant.
